Pharmacological action
Oktolipen - a metabolic agent, is an endogenous antioxidant that binds free radicals.
Thioctic acid is formed in the body by oxidative decarboxylation of alpha-keto acids. As a coenzyme of mitochondrial multienzyme complexes, it participates in the oxidative decarboxylation of pyruvic acid and alpha-keto acids.
Oktolipen helps to lower blood glucose and increase glycogen in the liver, as well as overcome insulin resistance. By the nature of the biochemical action, thioctic acid is close to B vitamins. Participates in the regulation of lipid and carbohydrate metabolism, stimulates cholesterol metabolism, and improves liver function.
Oktolipen has a hepatoprotective, lipid-lowering, hypocholesterolemic, hypoglycemic effect. Improves trophic neurons and axonal conduction, reduces the manifestations of diabetic and alcoholic polyneuropathy.
Indications
Diabetic polyneuropathy alcoholic polyneuropathy.
Contraindications
pregnancy
period of breastfeeding
children and adolescents under 18 years of age (efficacy and safety of use have not been established)
hypersensitivity to Oktolipen components.
Use during pregnancy and lactation
Use of the drug during pregnancy is contraindicated in the absence of sufficient clinical experience with thioctic acid during pregnancy.
Reproductive toxicity studies have not identified fertility risks, fetal development effects, or any embryotoxic properties of the drug.
The use of the drug Oktolipen® during lactation is contraindicated in the absence of data on the penetration of thioctic acid into breast milk.
Special instructions
In patients with diabetes mellitus, especially at the beginning of treatment with Oktolipen, frequent monitoring of blood glucose concentration is necessary. In some cases, a dose reduction of hypoglycemic agents is required.
During treatment, you must strictly refrain from drinking alcohol, as ethanol reduces the therapeutic efficacy of thioctic acid.
Composition
1 capsule contains tamsulosin hydrochloride 400 mcg, excipients: copolymer of methacrylic acid with ethyl acrylate (1: 1) dispersion 30%, microcrystalline cellulose, dibutyl sebactate, polysorbate 80f, silicon dioxide colloidal thioctic (? -lipoic) acid 300 mg.
Excipients: calcium phosphate disubstituted (calcium hydrogen phosphate) starch pregelatinized aerosil (silicon dioxide colloidal) magnesium stearate for the pharmaceutical industry.
Dosage and administration
Oktolipen in capsule form is prescribed orally at 600 mg (2 caps.) 1 time / day. Capsules are taken in the morning, on an empty stomach, 30 minutes before the first meal, without chewing, drinking plenty of water. The duration of the course of oral drug therapy is determined by the doctor.
Side effects of the
From the digestive system: symptoms of dyspepsia (including nausea, vomiting, heartburn).
Metabolism: symptoms of hypoglycemia (due to improved glucose uptake).
Allergic reactions: urticaria, systemic allergic reactions (including anaphylactic shock).
Drug Interactions
With simultaneous use with insulin and oral hypoglycemic agents, an increase in the hypoglycemic effect is observed.
After the morning intake of thioctic acid, iron and magnesium preparations, as well as dairy products (due to the calcium contained in them), it is recommended to take after lunch or in the evening. Ethanol reduces the therapeutic efficacy of thioctic acid.
Overdose
Symptoms: headache, nausea, vomiting. In severe cases (when receiving thioctic acid at a dose of 10–40 g or more than 10 tablets. 600 mg for adults, or at a dose> 50 mg / kg in children) - psychomotor excitation or clouding of consciousness, generalized seizures, severe disorders of acid-base balance with lactate acidosis, hypoglycemia (up to the development of coma), acute skeletal muscle necrosis, hemolysis, DVS, suppression of bone marrow activity, multiple organ failure.
Treatment: Immediate hospitalization and application of measures in accordance with the general principles adopted in the case of accidental poisoning (including induction of vomiting, gastric lavage, intake of activated charcoal) is recommended if a significant drug overdose is suspected. Treatment of generalized seizures, lactic acidosis and other life-threatening effects of intoxication should be carried out in accordance with the principles of modern intensive care and be symptomatic. There is no specific antidote for the drug. Hemodialysis, hemoperfusion or filtration methods forcibly removing thioctic acid from the body are ineffective.
