Indications

Prevention and treatment of calcium and / or vitamin D3 deficiency.

Addition to specific treatment and prevention of osteoporosis and its complications (bone fractures).

Contraindications

Hypercalcemia (increased concentration of calcium in the blood).

Hypercalciuria (elevated urinary calcium).

The drug in tablet form is not used in children under 3 years of age.

Calcium-D3 Nycomed contains aspartame, which in the body is transformed into phenylalanine. Therefore, patients with phenylketonuria should not take the drug.

Chewable tablets (orange, mint) contain sorbitol, isomalt and sucrose.

Chewable tablets (strawberry-watermelon) contain sorbitol and sucrose. The use is not recommended for patients with hereditary fructose intolerance, glucose-galactose malabsorption or sucrose-isomaltase deficiency.

Precautions

Pregnancy, lactation, renal failure.

Calcium carbonate with colecalciferol should be used with caution in patients with sarcoidosis, due to the risk of increasing the metabolism of vitamin D3 to its active form. In these patients, it is necessary to control the concentration of calcium in the blood serum and urine.

Use during pregnancy and lactation

Calcium-D3 Nycomed is used during pregnancy to supplement the deficiency of calcium and vitamin D3 in the body. The total daily intake during pregnancy should not exceed 2500 mg of calcium and 4000 IU of vitamin D. Hypercalcemia during overdose during pregnancy can have an adverse effect on the developing fetus.

Calcium-D3 Nycomed is used during lactation.

Calcium and vitamin D3 can pass into breast milk, so you should consider taking calcium and vitamin D from other sources in your mother and baby.

Special instructions

Serum calcium and creatinine should be monitored with prolonged therapy. Observation is especially important in elderly patients with simultaneous treatment with cardiac glycosides and diuretics (see the section “Interaction with other drugs and food products”) and in patients with an increased tendency to form kidney stones. In cases of hypercalcemia or signs of impaired renal function, the dose should be reduced or treatment should be discontinued.

Vitamin D3 should be taken with caution in patients with renal failure. In this case, it is necessary to control the content of calcium and phosphates in the blood serum. It is also necessary to take into account the risk of soft tissue calcification.

In order to avoid overdose, it is necessary to consider the additional intake of vitamin D and calcium from other sources.

Calcium and vitamin D3 should be used with caution in immobilized patients with osteoporosis due to the risk of developing hypercalcemia.

The simultaneous use of tetracycline or quinolone with antibiotics is usually not recommended, or should be carried out with caution (see section "Interaction with other medicines and food products").

Effect on the ability to drive vehicles and mechanisms

There is no data on the effect of the drug Calcium-D3 Nycomed on the ability to drive vehicles and mechanisms.

Dosage and Administration

Tablets can be chewed or dissolved and taken with food.

Prevention and treatment of calcium and / or vitamin D3 deficiency.

Adults and children over 12 years of age - 1 tablet 2 times a day, or 2 tablets once a day.

Children from 5 years to 12 years - 1-2 tablets per day.

Children from 3 to 5 years old - dosage in accordance with the doctor's recommendations.

Special patient groups

Patients with impaired liver function:

Dose adjustment is not required.

Patients with impaired renal function:

Calcium-D3 Nycomed should not be used in case of severe renal failure.

Elderly patients:

Dose is the same as for adults. A possible decrease in creatinine clearance should be considered.

Duration of treatment

When used for the prevention and treatment of calcium and / or vitamin D3 deficiency, the average duration of treatment is at least 4-6 weeks. The number of repeated courses throughout the year is determined individually.

When used to supplement specific therapy and prevent osteoporosis and its complications (bone fractures), the duration of treatment is determined individually by the doctor.

Metabolic and nutritional disorders:

Infrequent: hypercalcemia and hypercalciuria.

Very rare: Milk-alkaline syndrome (Burnett syndrome). It is usually noted only with an overdose (see section "Overdose").

Disorders of the gastrointestinal tract:

Rarely: constipation, flatulence, nausea, abdominal pain, diarrhea, dyspepsia.

Disorders of the skin and subcutaneous tissue:

Very rarely: itching, rash, urticaria.

Immune system disorders:

Frequency unknown: hypersensitivity reactions such as angioedema or laryngeal edema.

Drug interaction

Hypercalcemia can potentiate the toxic effects of cardiac glycosides when used together with calcium and vitamin D. It is necessary to control ECG and serum calcium.

Calcium preparations may decrease the absorption of tetracyclines from the gastrointestinal tract. Therefore, tetracycline drugs should be taken at least 2 hours before or 4-6 hours after taking Calcium-D3 Nycomed.

To prevent a decrease in the absorption of bisphosphonate preparations, it is recommended that they be taken at least one hour before taking Calcium-D3 Nycomed.

Glucocorticosteroids reduce calcium absorption, therefore, treatment with glucocorticosteroids may require an increase in the dose of Calcium D3 Nycomed.

With the simultaneous use of thiazide diuretics, the risk of hypercalcemia increases, because they increase tubular reabsorption of calcium. With the simultaneous use of thiazide diuretics, the serum calcium content should be regularly monitored.

Calcium reduces the effectiveness of levothyroxine, reducing its absorption. The period of time between doses of levothyroxine and Calcium-D3 Nycomed should be at least 4 hours.

Absorption of quinolone antibiotics decreases with simultaneous use with calcium preparations. Therefore, quinolone group antibiotics should be taken 2 hours before or 6 hours after taking Calcium-D3 Nycomed.

Calcium salts can reduce iron absorption, zinc and strontium ranelate. Therefore, iron, zinc or strontium ranelate preparations should be taken at least two hours before or two hours after taking Calcium - D3 Nycomed.

Treatment with orlistat may potentially interfere with the absorption of fat-soluble vitamins (e.g., vitamin D3).

Overdosing

An overdose can lead to hypercalcemia and hypervitaminosis D.

Symptoms of an overdose (hypercalcemia): anorexia, thirst, polyuria, muscle weakness, nausea, vomiting, constipation, abdominal pain, increased pain and fatigue nephrocalcinosis, urolithiasis and, in severe cases, cardiac arrhythmias. With prolonged use of excessive doses (over 2500 mg of calcium) - damage to the kidneys, soft tissue calcification.

Milk-alkali syndrome (Burnett syndrome) can occur in patients when ingested a large amount of calcium and well-absorbed alkaline solutions.

If signs of an overdose are found, you must stop taking calcium and vitamin D3, as well as thiazide diuretics and cardiac glycosides, and consult a doctor.

Treatment: gastric lavage, replenishment of fluid loss, use of loop diuretics (e.g. furosemide), glucocorticosteroids, calcitonin, bisphosphonates.

It is necessary to control the content of electrolytes in blood plasma, renal function and diuresis.

In severe cases, it is necessary to measure central venous pressure (CVP) and control an electrocardiogram (ECG).
