Pharmacological action

Triderm ointment is a combined preparation for external use.

Anti-inflammatory, vasoconstrictor, antipruritic, antibacterial, antifungal.

Ointment and Triderm cream combine the anti-inflammatory, antipruritic, anti-allergic and antiexudative effects of betamethasone dipropionate with the antifungal activity of clotrimazole and the broad antibacterial action of gentamicin sulfate.

Indications

- Dermatoses complicated by infections caused by pathogens susceptible to the drug, or if such infections are suspected, including simple and allergic dermatitis.

- Atopic dermatitis (including diffuse neurodermatitis).

- Limited neurodermatitis.

- Eczema.

- Dermatomycosis (dermatophytosis, candidiasis, lichen versicolor).

- Especially when localized in the inguinal region and large folds of the skin.

- Simple chronic lichen (limited neurodermatitis).

Contraindications

- Hypersensitivity.

- Cutaneous manifestations of syphilis.

- Chicken pox.

- Cold sores.

- Dermal post-vaccination reactions.

- Open wounds.

- Age to 2 years.

Special instructions

Do not use in ophthalmic practice.

Prolonged local use of antibiotics can sometimes lead to the growth of resistant microflora. In this case, as well as with the development of irritation, sensitization or superinfection with the use of Triderm, treatment should be interrupted and appropriate therapy should be prescribed.

Cross-allergic reactions with aminoglycoside antibiotics were observed. Any side effects encountered with systemic glucocorticosteroids, including inhibition of adrenal cortex function, may be observed with topical use of glucocorticosteroids, especially in children.

Systemic absorption of glucocorticosteroids or gentamicin with topical application will be higher if treatment is carried out on large surfaces of the body or when using occlusive dressings.

Avoid applying gentamicin to open wounds or skin lesions. Appropriate precautions should be observed in such cases, especially when treating children. Cancellation of the drug after prolonged use is recommended to be carried out gradually.

Use in pediatrics: children are prescribed from 2 years old only according to strict indications and under medical supervision, because possible development of systemic side effects associated with betamethasone.

When applying the Triderm, like many other topical glucocorticosteroids on large surfaces and / or with an occlusive dressing, it is possible to suppress the function of the hypothalamic-pituitary-adrenal system, a decrease in growth hormone excretion, as well as an increase in intracranial pressure, can be observed.

Composition of

1 g of ointment contains:

betamethasone 0.5 mg,

clotrimazole 10 mg,

gentamicin 1 mg.

Excipients:

liquid paraffin (50 mg),

white petrolatum (qs ad 1000).

Dosage and administration

Triderm ointment should be applied with a thin layer to the entire affected skin surface and surrounding tissue twice a day - in the morning and at night.

To ensure the effectiveness of treatment, Triderm ointment should be used regularly.

The duration of therapy depends on the size and location of the lesion, as well as the patient's response. If clinical improvement does not occur after 3-4 weeks of baking, this may serve as a reason for clarifying the diagnosis.

Side effects

Local reactions: burning sensation, erythema, exudation, pigmentation disorder, itching.

Adverse reactions due to betamethasone: burning sensation, itching, irritation and dry skin, folliculitis, hypertrichosis, acne eruptions, hypopigmentation, perioral dermatitis, maceration of the skin, secondary infection, atrophy of the skin, striae, prickly heat.

Adverse reactions due to clotrimazole: erythema, desquamation, local edema, pruritus, urticaria, skin maceration, paresthesia.

Adverse reactions due to gentamicin: hyperemia, pruritus.

Overdose

Symptoms: with prolonged use of local corticosteroids in high doses, suppression of adrenal function with the development of secondary adrenal insufficiency and symptoms of hypercorticism, including Cushing's syndrome, is possible.

An overdose of clotrimazole when applied topically does not lead to any symptoms. With a single overdose of gentamicin, the appearance of any symptoms is also not expected.

Long-term treatment with high doses of gentamicin can lead to the growth of insensitive flora.

Treatment: symptomatic. The acute symptoms of hypercorticism are usually reversible. If necessary, carry out the correction of electrolyte imbalance. In case of chronic corticosteroids toxicity, gradual withdrawal of corticosteroids is recommended.
