Pharmacological action

GlucaGen 1 mg HypoKit contains a genetically engineered human glucagon, a protein-peptide hormone, a physiological insulin antagonist involved in the regulation of carbohydrate metabolism. Glucagon enhances the breakdown of glycogen in the liver to glucose-6-phosphate (glucogenolysis), resulting in an increase in the concentration of glucose in the blood. Glucagon is not effective in treating patients in whose liver glycogen stores are depleted. For this reason, glucagon is ineffective or not effective at all in the treatment of fasting patients or patients with adrenal insufficiency, chronic hypoglycemia or hypoglycemia caused by alcohol intake. Unlike adrenaline, glucagon does not affect muscle phosphorylase and therefore cannot facilitate the transfer of carbohydrates from skeletal muscle richer in glycogen stores. Glucagon stimulates the release of catecholamines. In the presence of pheochromocytoma, glucagon can provoke the release of a large number of catecholamines by the tumor, which cause a sharp increase in blood pressure. Glucagon reduces the contractility of smooth muscles of the gastrointestinal tract. The action of the drug begins 1 minute after intravenous injection, the duration of the drug is 5-20 minutes, depending on the dose and organ. In the treatment of severe hypoglycemia, the effect of glucagon on blood glucose is usually observed within 10 minutes.

Indications

Severe hypoglycemic conditions (low blood glucose) occurring in patients with diabetes mellitus after insulin injection or taking hypoglycemic tablets.

Contraindications

Hypersensitivity to glucagon or any other component of the drug hyperglycemia pheochromocytoma.

Use during pregnancy and lactation

GlucaGen does not pass through the human placental barrier and can be used to treat severe hypoglycemia during pregnancy. When prescribing the drug during breastfeeding, there was no risk to the child.

Composition

Active substance: glucagon hydrochloride, genetic engineering - 1 mg (corresponding to 1 IU).

Dosage and administration

For the preparation of an injection solution, 1 mg (1 ME) of lyophilisate is dissolved in 1 ml of solvent. The resulting solution can be administered subcutaneously, intramuscularly or intravenously. In this dosage form, GlucaGen 1 mg HypoKit is administered subcutaneously or intramuscularly. Administration of the drug by medical personnel 1 mg (adults and children with a body weight of more than 25 kg or 6-8 years old) or 0.5 mg (children with a body weight of less than 25 kg or less than 6-8 years old) are injected subcutaneously, intramuscularly or intravenously. The patient usually regains consciousness within 10 minutes after administration of the drug. After the patient regains consciousness, he must be given food rich in carbohydrates to prevent the re-development of hypoglycemia. If within 10 minutes the patient does not regain consciousness, he needs to inject glucose intravenously. Administration of the drug to the patient by relatives (s) Relatives or close friends of the patient with diabetes should know that if he develops a severe hypoglycemic reaction, he needs medical assistance. If a patient with diabetes develops severe hypoglycemia and is unable to eat sugar, relatives or friends should give him an injection of GlucaGen 1 mg HypoKit. Enter 1 mg (adults and children weighing more than 25 kg) or 0. 5 mg (for children weighing less than 25 kg or younger than 6-8 years) subcutaneously or in the upper outer part of the thigh muscles. The patient usually regains consciousness within 10 minutes after administration of the drug. After the patient regains consciousness, he needs to be given sugar to prevent the re-development of hypoglycemia. All patients with severe hypoglycemia need medical attention.

Side effects

From the digestive tract: nausea, vomiting.

From the cardiovascular system and blood (blood formation, hemostasis): transient increase in blood pressure, tachycardia.

Allergic reactions: skin rash and itching, urticaria, bronchospasm, anaphylactic shock.

Other: hypokalemia, dehydration.

Drug Interaction

On the background of beta blockers, the introduction of the drug GlukaGen 1 mg HypoKit can lead to marked tachycardia and increased blood pressure. Insulin: The action of glucagon is the opposite of that of insulin (insulin glucagon antagonist). Indomethacin: When used together, glucagon may lose the ability to increase blood glucose and even cause hypoglycemia. Warfarin: When used together, glucagon can enhance the action of warfarin's anticoagulant.

Overdose

GlucGen 1 mg HypoKit overdose may cause nausea, vomiting, diarrhea, hypokalemia, tachycardia, hypertension. Treatment is symptomatic. It is necessary to constantly monitor the level of potassium and, if necessary, its correction. The use of forced diuresis and hemodialysis is ineffective. In case of vomiting - rehydration and recovery of potassium losses.

Storage conditions

GlucaGen (in powder form) should be stored at a temperature not exceeding 25 РC. Do not freeze to prevent damage to the syringe. The bottle with the drug GlukaGen should be stored in a dark place. The finished solution of the drug GlucaGen 1 mg HypoKit should be used immediately after preparation. Do not store ready-made solution for later use. Keep out of the reach of children.
