Pharmacological action

A.06.AD11 Lactulose

improves absorption of phosphates and Ca2 + salts, promotes the excretion of ammonium ions.

Lactulose is broken down by the intestinal flora of the colon into low molecular weight organic acids, which leads to a decrease in pH and an increase in osmotic pressure and, as a result, an increase in the volume of intestinal contents. These effects stimulate intestinal motility and have an effect on stool consistency. As a result, the physiological rhythm of the colon emptying is restored.

In hepatic encephalopathy, the effect is attributed to the suppression of proteolytic bacteria by increasing the number of acidophilic bacteria (e.g., lactobacilli), the conversion of ammonia to ionic form due to acidification of the contents of the colon, bowel movement due to lower pH in the colon and osmotic effect, as well as reducing nitrogen-containing toxic substances by stimulating bacteria utilizing ammonia for bacterial protein synthesis.

Lactulose as a prebiotic substance enhances the growth of beneficial bacteria, such as bifidobacteria and lactobacilli, while it becomes possible to suppress the growth of potentially pathogenic bacteria, such as Clostridium and Escherichia coli, which provides a more favorable balance of the intestinal flora.

Pharmacokinetics:

Absorption is low. After ingestion, it reaches the large intestine unchanged, where it is split by the intestinal flora. Fully metabolized when used in doses up to 40-75 ml. When used in higher doses, it is partially excreted unchanged.

Indications

- Constipation: regulation of the physiological rhythm of colon emptying.

- Stool softening for medical purposes (hemorrhoids, conditions after surgery on the colon and in the anus).

- Adult hepatic encephalopathy: treatment and prevention of hepatic coma or precoma.

Contraindications

- galactosemia

- obstruction, perforation or risk of perforation of the gastrointestinal tract

- hypersensitivity to any component of the

preparation - intolerance to galactose or fructose, galactose or glucose-lactase deficiency.

With caution:

- rectal bleeding undiagnosed

- colostomy, ileostomy

- liver disease, alcoholism, epilepsy, pregnancy, breastfeeding, children due to the fact that the drug contains ethanol in the flavor.

Use during pregnancy and lactation

No effect on the fetus or infant is expected since the systemic effect of lactulose on the pregnant or lactating woman is negligible. DuphalacВ® can be prescribed during pregnancy and during breastfeeding.

It must be borne in mind that the drug Duphalac in the form of a syrup with plum flavor contains 0, 4% ethanol (see Special Instructions).

No effect on reproductive function is expected since the systemic effects of lactulose are negligible.

Special instructions

In case of abdominal pain of unknown origin before starting treatment or lack of therapeutic effect a few days after starting to take the drug, you should consult your doctor.

Please note that Dufalac® may contain minor amounts of bound sugars (e.g. lactose, galactose, epilactose and fructose).

When taking the recommended doses for treating constipation, sugar should not be a problem for patients with diabetes. In the treatment of hepatic encephalopathy, higher doses of the drug are usually prescribed, and the sugar content in it should be considered for patients with diabetes mellitus.

The residual sugars present in Dufalac® are approximately 0, 075 XE in 5 ml of syrup.

Prolonged use of doses exceeding those recommended in the instructions or improper use can lead to diarrhea and impaired water-electrolyte balance. Duphalac® in the form of a syrup with plum flavor contains 0.4% ethanol (ethanol) in the composition of the flavoring, that is, up to 160 mg in the maximum daily dose taken in the treatment of constipation, which corresponds to 4 ml of beer or 2 ml of wine. This may be unfavorable for patients with alcoholism. The alcohol content must be considered when used during pregnancy and during breastfeeding, as well as in children and patients at high risk with liver disease or epilepsy.

In the treatment of children, laxatives should be used in exceptional cases and under the supervision of a physician. Should be considered, that during treatment, emptying reflex disorders may occur.

Impact on the ability to drive transp. Wed and fur .:

The use of the drug Dufalac® does not affect or has a slight effect on the ability to drive a car and mechanisms.

Side effects

In the first days of taking lactulose, flatulence may occur. As a rule, it disappears after a few days.

In the case of high doses for a long time in the treatment of hepatic encephalopathy, the patient may develop disturbances in the water-electrolyte balance due to diarrhea.

Drug Interactions

No other drug interaction studies have been conducted.

Overdose

Symptoms: Abdominal pain and diarrhea are possible with a very high dose.

Treatment: discontinuation of the drug or dose reduction. In case of large fluid loss due to diarrhea or vomiting, correction of disturbances in the water-electrolyte balance may be required.
