Weak or moderate pain syndrome with spasms of smooth muscles of internal organs: renal, hepatic, biliary and intestinal colic, biliary dyskinesia, algomenorrhea.
For short-term symptomatic treatment: arthralgia, neuralgia, myalgia, sciatica.
As an auxiliary drug: pain syndrome after surgical interventions and diagnostic procedures.
If necessary, the drug can be used to reduce elevated body temperature in case of colds and infectious and inflammatory diseases (tablets).
Tablets: inside (better after meals), adults and children over 15 years old usually - 1-2 tablets. 2-3 times a day. The daily dose should not exceed 6 tablets. Duration of admission is no more than 5 days. An increase in the daily dose or duration of treatment is possible only on the recommendation and under the supervision of a physician.
In children, the drug is used only as directed by a doctor.
Children 6Ц8 years old - 1/2 tab., 9Ц12 years old - 3/4 tab., 13Ц15 years old - 1 tab. 2-3 times a day.
Tablets are white, round, flat, chamfered and scored.

Excipients: lactose, starch, silicon dioxide, talc, magnesium stearate.
Hypersensitivity to pyrazolone derivatives (butadione, tribuzone), other components of the drug,
oppression of bone marrow hematopoiesis,
stable and unstable angina pectoris,
chronic heart failure in the stage of decompensation,
severe liver or kidney dysfunction,
deficiency of glucose-6-phosphate dehydrogenase,
tachyarrhythmia,
acute 'intermittent' porphyria,
angle-closure glaucoma,
hyperplasia of the prostate gland (with clinical manifestations),
intestinal obstruction,
megacolon,
collapse,
granulocytopenia,
pregnancy (especially the first trimester and the last 6 weeks),
breastfeeding period,
children's age (up to 5 years - tablets, up to 3 months or with a body weight <5 kg - injection solution).
With caution and under the supervision of a physician - for patients with impaired liver or kidney function, with a tendency to arterial hypotension (SBP <100 mm Hg), bronchospasm, bronchial asthma, increased individual sensitivity to NSAIDs or non-narcotic analgesics (incl. History of aspirin triad).
For children and adolescents under 18 years of age, the drug should be used only as directed by a doctor.
pharmachologic effect
Combined drug with analgesic and antispasmodic action.
Metamizole sodium - an analgesic-antipyretic, is a pyrazolone derivative. It has analgesic, antipyretic and anti-inflammatory effects.
Pitofenone hydrochloride has a direct myotropic effect on the smooth muscles of internal organs and causes its relaxation.
Fenpiverinium bromide, due to its m-anticholinergic action, has an additional relaxing effect on smooth muscles.
The combination of the three components of the drug leads to a mutual enhancement of their pharmacological action, expressed in pain relief, smooth muscle relaxation and fever reduction.
Application during pregnancy and lactation
The drug is contraindicated for use during pregnancy (I trimester and the last 6 weeks) and during lactation (breastfeeding).
Application for violations of liver function
Contraindicated in severe liver dysfunction. With caution and under the supervision of a physician, the drug should be prescribed for violations of liver function.
Application for impaired renal function
Contraindicated in severe renal impairment. With caution and under the supervision of a physician, the drug should be prescribed for impaired renal function.
special instructions
With prolonged (more than 1 week) use of the drug, it is necessary to monitor the peripheral blood picture (the number of leukocytes and platelets) and the functional state of the liver. If agranulocytosis or thrombocytopenia is suspected, use of Maxigan should be discontinued.
Solution for injection is not used to relieve acute abdominal pain until the cause is clarified.
Parenteral administration of the drug is used in case of emergency (for example, renal or hepatic colic) and in cases where oral administration of the drug is not possible. With caution, intravenous administration of the drug should be carried out (since there is a risk of arterial hypotension). IV injection should be carried out slowly, the patient should be in the supine position, it is necessary to monitor blood pressure, heart rate and respiratory rate.
When using the drug Maxigan, alcohol should be avoided.
X-ray contrast agents and colloidal blood substitutes should not be used during treatment with drugs containing metamizole sodium.
Influence on the ability to drive vehicles and mechanisms
During the period of taking the drug, it is not recommended to drive vehicles and engage in other potentially hazardous activities that require the speed of psychomotor reactions.
Overdose
Symptoms: vomiting, decreased blood pressure, drowsiness, confusion, nausea, dry mouth, epigastric pain, changes in sweating, impaired liver and kidney function, convulsions.
Treatment: gastric lavage, the appointment of activated carbon, if necessary, symptomatic therapy is carried out.
Drug interactions
With the simultaneous use of Maxigan with other non-opioid analgesics, a mutual increase in toxic effects is noted.
Tricyclic antidepressants, oral contraceptives, allopurinol disrupt the metabolism of metamizole sodium and increase its toxicity.
Barbiturates, phenylbutazone and other inducers of liver microsomal enzymes weaken the effect of sodium metamizole.
With the simultaneous use of Maxigan with cyclosporine, the level of the latter in the blood decreases.
Sedatives and tranquilizers enhance the analgesic effect of metamizole sodium.
With the simultaneous appointment of Maxigan with blockers of histamine H1 receptors, butyrophenones, phenothiazines, amantadine and quinidine, an increase in the m-anticholinergic action is possible.
With simultaneous use with ethanol, a mutual enhancement of the effects is noted.
With the simultaneous use of Maxigan with chlorpromazine or other phenothiazine derivatives, severe hyperthermia may develop.
Metamizole sodium, displacing oral hypoglycemic drugs, indirect anticoagulants, GCS and indomethacin from the connection with the protein, can enhance their clinical effects.
With the simultaneous use of Maxigan with thiamazole and cytostatics, the risk of developing leukopenia increases.
With the simultaneous use of codeine, blockers of histamine H2 -receptors, propranolol slows down the inactivation of sodium metamizole and enhances its effect.
Pharmaceutical interactions
Solution for injection is pharmaceutically incompatible in one syringe with solutions of other drugs.
