Pharmacological action of

Mycophenolate Mofetil-T has an immunosuppressive effect. Selectively inhibits inosine monophosphate dehydrogenase and inhibits the synthesis of guanosine nucleotoid. It has a pronounced cytostatic effect on lymphocytes, inhibits their proliferation, also inhibits the formation of antibodies by B-lymphocytes. When taken orally, it is rapidly absorbed and completely hydrolyzed, forming mycophenolic acid. It is excreted mainly by the kidneys. In combination with azathioprine and cyclosporine, as well as with the induction course of antithymocytic globulin, reduces the frequency of adverse outcomes in the first 6 months after transplantation.

Indications

Prevention of rejection reaction and treatment of allogeneic renal transplant rejection (in combination with cyclosporin A and glucocorticoids).

Contraindications

Hypersensitivity, exacerbation of peptic ulcer of the stomach and duodenum and other gastrointestinal diseases, pregnancy, lactation, children.

Special instructions

It is necessary to take into account the possibility of increasing the risk of developing lymphoproliferative processes, regularly monitoring the composition of peripheral blood, with a decrease in the number of neutrophils, a dose reduction or a break in treatment (and appropriate therapy) is indicated. Composition

Active substances: mycophenolate mofetil 500 mg.

Dosage and administration

Used as part of combination therapy with cyclosporine and corticosteroids. To prevent the transplant rejection reaction, the first dose of mycophenolate mofetil is taken during the first 3 days after surgery. When administered, the recommended dose is 2 g per day, the frequency of administration - 2 times a day. For the treatment of rejection reactions - 3 g per day.

Side effects

Nausea, vomiting, diarrhea, constipation, leukopenia or leukocytosis, anemia, thrombocytopenia, increased incidence of infectious and septic complications headaches, insomnia, tremor, fever, asthenia, lumbago, hypertension, cough, pharyngitis, dyspnea, peripheral edema, oropharyngeal candidiasis, hematuria, tubular necrosis, impotence, acne, hyper- or hypokalemia, hyperglycemia, hypophosphatemia, hypercholesterolemia, allergic reactions, rash, skin itching.

Drug interactions

Probenecid and other drugs affecting tubular secretion increase the plasma concentration of mycophenolic acid. Antacids containing hydroxides of magnesium and aluminum, reduce absorption. Drugs that alter the flora of the gastrointestinal tract may interfere with enterohepatic circulation. Reduces the effectiveness of oral contraception. Azathioprine increases the risk of developing lymphomas, especially the skin.

Overdose

Symptoms: an increase in the incidence of gastrointestinal and hematological (in particular neutropenia) side effects.

Treatment: Dose reduction or withdrawal of the drug. To accelerate excretion, the appointment of bile acid sequestrants is possible. Small amounts of glucuronide can be removed by hemodialysis (mycophenolic acid is not dialysable).
