pharmachologic effect

Okumetil is a combined preparation containing zinc sulfate, diphenhydramine and naphazoline. When applied topically, zinc sulfate has an astringent, drying, anti-inflammatory and antiseptic effect. Naphazoline is a sympathomimetic. It has a fast, pronounced and prolonged vasoconstrictor effect. When applied topically, it reduces swelling and hyperemia of the mucous membranes. Diphenhydramine is a blocker of H1-histamine receptors. When applied topically, it reduces capillary permeability and prevents tissue edema. Thus, Okumetil has antiseptic, anti-allergic and anti-inflammatory effects.

Pharmacokinetics

When applied topically, it undergoes systemic absorption. There is no information on the degree of penetration into various tissues of the eye after topical application.

Side effect

After instillation, a slight burning sensation, blurred vision, and the development of allergic reactions may occur. With aphakia, the development of maculopathy and the appearance of central scotoma are possible. In this case, withdrawal of the drug leads to the reverse development of symptoms. After local use of naphazoline, which is part of Okumetil, reactive hyperemia and edema of the mucous membrane of the eyes, pupil dilation, increased IOP (intraocular pressure) may occur. With prolonged use, the phenomenon of tissue irritation occurs most often. Since, when applied topically, naphazoline has a general resorptive effect, it is possible to develop tachycardia, increased blood pressure, nausea, and headache. With local application of diphenhydramine, side effects of both local and systemic nature may develop: accommodation paresis, photosensitivity, dryness and numbness of the mucous membrane of the eyes, drowsiness, weakness, decreased psychomotor reactions and impaired coordination of movements, dizziness. With prolonged use of zinc salts, dryness of the mucous membrane of the eyes is possible.

special instructions

To avoid contamination of the solution, keep the vial tightly closed and avoid touching the pipette tip with any surface.

Influence on the ability to drive vehicles and use mechanisms

Within 10-15 minutes after installation, a decrease in visual acuity is possible, and therefore, care must be taken when driving a car and when working with mechanisms.

Overdose

There are no data on drug overdose.

Drug interactions

Since a mutual decrease in the effects of diphenhydramine and drugs that stimulate the central nervous system is possible, it is not recommended to use the drug Okumetil in patients using these drugs. Okumetil should not be used concomitantly with MAO inhibitors and within 10 days after stopping them. As a result of the combined use of beta-blockers, MAO inhibitors with adrenergic agonists, the action of the latter is enhanced. Naphazoline slows down the absorption of local anesthetics, therefore, the combined use of Okumetil with local anesthetics used in ophthalmology is not recommended. Zinc sulfate is pharmaceutically incompatible with salts of silver, lead, quinine, ichthyol, citral, protargol, alkaline-reacting substances.
